  • Introduction : La forte consommation d inhibiteurs de la pompe à protons (IPP) a un impact majeur sur les dépenses de l Assurance-Maladie. Bien tolérés, ils sont souvent prescrits en dehors des indications de l autorisation de mise sur le marché (AMM). Nous avons réalisé un audit à partir d une étude connaissances attitudes et pratiques, associée à une action de formation, sur la prescription des IPP d un échantillon de médecins généralistes militaires. Nous avons, au préalable, étudié le volume de prescriptions des IPP dans les Armées.Méthode : Nous avons déterminé le nombre de prescription d IPP et leur rang dans l ensemble des médicaments prescrits par les médecins militaires durant la période 2009-2010 d après les données de la Caisse Nationale Militaire de Sécurité Sociale (CNMSS). Notre audit a évalué les pratiques de 20 médecins d unités d Ile-de-France à l aide de deux questionnaires hétéro-administrés encadrant une action de formation.Résultats : Les IPP arrivaient au 6ème rang des médicaments les plus prescrits par les médecins militaires selon les données de la CNMSS. On notait un manque de connaissance global des recommandations, ainsi, plus de la moitié des prescriptions d IPP concernant la prévention des lésions gastro-intestinales induites par des AINS n étaient pas conforme à l AMM. Seuls 25 % des médecins prescrivaient correctement l IPP en cas de RGO. Les des praticiens prescrivaient un IPP pour des manifestations extra-digestives isolées du RGO. L action de formation a permis une amélioration notable des résultats.Discussion : L enquête réalisée auprès de la CNMSS et le manque de connaissance des recommandations suggèrent l importance des actions de formations
  • Introduction: The extensive consumption of proton pump inhibitors (PPIs) is one of the main spendings of Healthcare. They are well tolerated and therefore often prescribed even when they don t correspond to the indications of the full market approval (AMM). We have carried out an audit of the prescription practices of the PPIs by a sample of military general practitioners (GPs) before and after an action of course training. Beforehand, we have evaluated the number of prescriptions of PPIs in the army.Method: We have determined the number of PPIs prescriptions and their rank among the medicines prescribed by practitioners in the army in 2009-2010, according to the data provided by the French Military Social Security Fund (CNMSS). Therefore our audit has assessed the practices of 20 unit practitioners in Ile-de-France through two questionnaires before and after a training scheme.Results: The PPIs were the 6th most prescribed medicine by practitioners in the army. We noted a global lack of knowledge of the recommendations: over one of the PPIs prescriptions aiming at preventing gastro-intestinal lesions resulting from the non-steroidal anti-inflammatory did not conform with AMM. Only 25% of the GPs adequately prescribed PPIs in case of GERD. 75% of them prescribed PPIs for extra-digestive manifestations distinct from the GERD. The further training has led to a clear improvement in these results. Discussion : The study conducted with the CNMSS and the lack of knowledge of the recommendations leads us to think that it seems necessary to go on with course training projects for military GPs, focusing on the prescription of PPIs. To generalize and improve this action, it would be advisable to use the same questionnaire for the 2 interviews of audit and to appoint a doctor referent by territory for the delivery and the explanation of the guide of good use
